Maintenance & Website Change Request Form
Please use the form below to submit requests for changes to your web site or email account.
If you are requesting updates be made to multiple web sites, fill out one form per site.
For changes to your web site, please include the name of the page or form where the changes are required. In most browsers, the page name can be found at the top of the browser window.
If multiple files are required to complete the requested change, then these files will need to be packaged as a single Zip. You may use your favorite Zip utility, or you may download and use the following free Zip utility;

If you already have a document describing the changes, you may include these in the Zip file. If you do this, please indicate on the form that the instructions have been included in the Zip file.
This form is not mandatory, however it may reduce your web site development/design expenditures, if you are on a tight budget. By filling out the form thoroughly, you enable ThinkTech Design to get started on your modification without much research, right away. This will save ThinkTech Design time, and therefore reduce time for which you will be billed.
